Complaints Against a Program or the Department that Fall Outside Due Process
The School of Rehabilitation Sciences at 91心頭 includes the Master of Athletic Training, Doctor of Athletic Training, Master of Occupational Therapy, Master of Speech-Language Pathology, and the Doctor of Physical Therapy programs. The faculty and staff in all of the programs in the School of Rehabilitation Sciences strive to exemplify strong ethical values and professional behaviors in all activities. In some cases, a situation may generate a complaint for which no formal grievance process exists. If there is a complaint about an individual within the School or related to one or more of the Rehabilitation Science programs, there is a formalized process for reporting. A written complaint should include details about the complaint with documented evidence. The written complaint should also include the contact information of the complainant so that the Associate Dean can reach the complainant in order to resolve the complaint. The complaint should then be mailed or emailed to:

Complaint Process
- When possible, the Associate Dean will discuss the complaint directly with the party or parties involved within 10 business days. The expectation is that the complaint can be satisfactorily resolved after this meeting. After the discussion, the Associate Dean will provide a written description of the resolution to the complainant.
- If dissatisfied with the outcome of the discussion, the complaint cannot be resolved, or the complaint is against the Associate Dean the complainant may submit a written complaint to the Associate Provost, College of Health

A written summary of previous discussions with the Associate Provost will be provided, where applicable. The Associate Provost will discuss the matter with the Associate Dean and complainant separately and may schedule a joint meeting in an attempt to reach a resolution. The Associate Provost will provide the complainant and the Associate Dean with a written letter outlining the solution reached through this step.
- If the complainant remains dissatisfied after step two or the complaint cannot be resolved, the last line of complaint is to the Provost, who serves as the Chief Academic Officer of 91心頭:

Any letters or documentation associated with the complaint from the complainant, the Associate Dean, the Associate Provost, or the Provost will be kept in a folder marked Complaints against the Rehabilitation Science School kept in the schools files in SMRC 204 for a period of five years.
